Hospital to Home Program Shows Early Success in Windsor–Essex

The Hospital to Home (H2H) program in Windsor–Essex is already delivering meaningful results for patients transitioning from hospital to home, with SE Health playing a central role in delivering coordinated, in-home care and community supports.

Supported by the province’s 1.1-billion dollar investment in home care, the H2H program provides up to 16 weeks of interdisciplinary, in-home services for eligible patients who no longer require acute care but still need clinical and rehabilitative support after discharge. SE Health teams deliver integrated care that includes nursing, personal support, therapy, social work, and dietetic services, helping people recover safely and independently at home.

Since onboarding its first participant in October 2025, the program has registered more than 115 patients and is on track to support over 230 by the end of March 2026.

“We are thrilled to support this innovative and integrated model that is redefining how we deliver authentic, people centered care,” said John Yip, President and CEO of SE Health. “By expanding services and strengthening programs like Hospital to Home, we reduce system pressure, shorten lengths of stay, and improve patient flow while delivering high quality care where people want it most.”

The program is delivered in partnership with Windsor Regional Hospital as part of Ontario’s broader effort to expand access to care at home.
