Hospital To Home
Our @Home Program delivers intensive, patient-centred care for medically complex seniors, bridging hospital to home with integrated medical, mental health, housing, and social supports. This holistic model promotes recovery, independence, and sustained wellness while easing pressure on acute care.
Transitional and Restorative Care Units
We offer short-term stays in retirement homes for patients discharged from acute care who need reactivation and recovery before returning home. Through interdisciplinary care, we improve patient flow, reduce ALC rates, and shift care from hospitals to the community – enhancing system efficiency and patient outcomes.
Care Management
Personalized whole-person assessments, care coordination & navigation services by our expert nurses, to better manage care, and enhance the quality and value of homecare services. Through warm handoffs, early community support services integration, and evidence-based tools, we ensure timely, personalized and equitable care.
