  • Dietetics

    Dietetics focuses on an individual's nutritional needs and eating habits. Registered Dietitians are valuable members of the health care team, providing health promotion, disease prevention, treatment and supportive care.

    Registered Dietitians provide diet assessments, develop nutritional plans and menus, and support individuals and families with making dietary changes. Dietitians are especially helpful for patients with cancer, diabetes, digestive, or swallowing disorders.

    Our Registered Dietitians provide services in a variety of settings including homes, hospitals and long-term care homes. We can provide:

    • Development, implementation and evaluation of nutrition plans;
    • Education and counselling for individuals, families and caregivers;
    • Assessment of nutrient/diet/drug interactions, and counselling to minimize related adverse implications;
    • Pediatric nutrition care services at home and in school;
    • Consultation with health care teams and written documentation of activities; and
    • Support for seniors who have swallowing difficulties.
  • Home Support

    Getting older, illness, or recovering from surgery can make it difficult to keep up with housekeeping or get out to the store. Whether you need a little help at home or a trusted care provider to stay with your loved one while you are away, our compassionate staff can help people maintain their independence within the comfort of their own homes.

    One of our case managers (specialized nurses) will visit you at home to conduct a free assessment of your needs and work with you to create a home care plan. We are dedicated to matching our staff to your individual needs and personality. Consistency of care is paramount to our service. Whenever possible, the same staff will provide care at each visit, on a schedule that is convenient for you.

    Our caring staff will visit your home to support you by helping with a variety of tasks, including:

    • Meal planning and preparation;
    • Grocery shopping;
    • Laundry;
    • Light housekeeping;
    • Transporting and/or accompanying you to and from medical appointments, shopping or other daily activities; and
    • Social interaction, such as card games, reading, walks and conversation.
  • Personal Care

    Personal Support Workers (PSWs) and Community Health Workers (CHWs) assist people with daily personal care needs including bathing, toileting, dressing, mobility and feeding as well as other activities of daily living. Our PSWs are graduates of certified, accredited educational programs.

    Individuals rely on PSWs to help them stay at home, to be safe and comfortable, and to live their lives with dignity.

We are dedicated to matching our staff to your individual needs and personality. Consistency of care is paramount to our service. Whenever possible, the same staff will provide care at each visit, on a schedule that is convenient for you.

    Our compassionate, trained staff will visit your home to help with a variety of tasks, including:

    • Bathing, personal grooming;
    • Toileting;
    • Assistance with dressing;
    • Medication reminders;
    • Meal planning and preparation;
    • Assistance with feeding;
    • Exercise activities and mobility assistance (lifts, transfers);
    • Grocery shopping;
    • Laundry;
    • Light housekeeping; and
    • Transportation to and from activities of daily living.
  • Senior Care

    Senior care services provide a range of health care and support services to older adults who have acute, chronic (ongoing), palliative or rehabilitative health care needs. Our care is based on best clinical practices and evidence.

    You may require senior care services if you:

    • Have health issues or life changes related to aging that require more care than you or your family can provide;
    • Need support to continue living safely at home; and/or
    • Are released from hospital and need short-term care.

    We understand that many seniors want to continue living in their homes as independently as possible. A wide range of professionals with specialized experience in senior care, rehabilitation and personal support services (such as help with bathing and meals) are available in your home.

    Our services designed to support seniors include:

    • Professional nursing care in your home. This may include monitoring and support for aging issues common to older adults, and managing multiple conditions (e.g. Alzheimer's Disease/dementia, cancer, depression, hip fracture and stroke);
    • Screening for age-related illnesses, cognitive and emotional changes;
    • Falls prevention and risk assessment;
    • Personal care and home support to assist you in your daily living activities including bathing, dressing, walking and exercise;
    • Nutrition support.

    Our goal is to help seniors be as healthy and independent as possible, living safely in their preferred environment. We work collaboratively with older adults, their families, and a variety of healthcare professionals to:

    • Promote health and optimal care;
    • Enhance quality of life;
    • Prevent disease, injury, and excess disability;
    • Maintain seniors' desired level of independence;
    • Promote rehabilitation;
    • Provide palliative care to older adults; and
    • Support family caregivers.
  • Transportation

    Our caring staff will provide transportation in safe, reliable vehicles whenever you need it. We can drive you to medical appointments, shopping, banking, errands or other activities. We will also be waiting for you when your appointment is finished to ensure you are taken home safely.
